Medicare Part D Information

Search AdvantageCare Medicare Part D formulary and participating Pharmacy locations. Download appropriate Injectable and Part D prior authorization forms. Members will need approval from AdvantageCare for those drugs listed as requiring prior authorization before the drug may be covered.
